Position Overview

The Bilingual Social Services Coordinator operates under the supervision of the Social Services Management team. This role is essential in engaging with members of the Blue Shield of California Promise Health Plan to assess their social determinants of health and implement comprehensive support strategies.

Key Responsibilities
  • Outreach and Assessment: Conduct outreach to members, performing detailed assessments to identify social needs and barriers to healthcare access.
  • Resource Linkage: Facilitate connections to community resources aimed at alleviating social obstacles, thereby enhancing members' ability to manage their healthcare effectively.
  • Collaboration: Work closely with healthcare providers to align treatment plans with members' care requirements.
  • Case Management: Efficiently oversee a caseload of individuals with complex needs, including those facing chronic homelessness or severe mental health challenges.
  • Service Coordination: Coordinate with state and community programs to ensure comprehensive support for members, particularly those who may struggle with independent navigation of services.
  • Field Engagement: Conduct visits in various settings, such as healthcare facilities and community organizations, to perform assessments and deliver psychosocial support.
  • Documentation: Maintain thorough and timely documentation of assessments and referrals in accordance with established protocols.
Qualifications
  • Education: A Master's Degree in Social Work (MSW) or a related discipline from an accredited institution is required.
  • Experience: A minimum of three years of experience working with the Medi-Cal population or in medical social work settings is essential.
  • Assessment Skills: Proven ability to conduct comprehensive psychosocial assessments and develop tailored care plans for diverse client populations.
  • Technical Proficiency: Familiarity with software applications such as Microsoft Excel, Outlook, Word, and PowerPoint is necessary.

This position plays a critical role in supporting the health and well-being of members by addressing their social needs and facilitating access to necessary resources.
