Job Title: LPN - Long Term Care/Transitional Care

We are seeking a skilled LPN to join our team at a Long Term Care/Transitional Care facility in Seattle, WA. The ideal candidate will have at least 1 year of experience in skilled long term care/transitional care and be able to work 8hr days.

Responsibilities:
  • Provide high-quality patient care in a long term care/transitional care setting
  • Work collaboratively with the healthcare team to ensure seamless care delivery
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements:
  • 1 year of experience in skilled long term care/transitional care
  • Current LPN licensure in the state of Washington
  • Ability to work 8hr days, including every other weekend
  • Reliable transportation and a valid driver's license
