Job Description:

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse. As a Traveling Healthcare Professional, you will have the opportunity to work with a variety of patients and families, providing high-quality care and support in the comfort of their own homes.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
  • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to meet their unique requirements
  • Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination
  • Communicate effectively with patients, families, and caregivers, providing education and support to promote healthy outcomes
  • Document patient progress, identifying areas for improvement and implementing changes as needed

Requirements:

  • Current RN licensure in the state of Wisconsin
  • Minimum 1 year of experience in home health or a related field
  • Strong assessment and communication skills, with ability to work independently and as part of a team
  • Ability to lift 25 pounds and stand for long periods, with frequent walking and bending
