Job Summary

The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. The ACN utilizes a collaborative approach to anticipate, integrate, and coordinate patient care plans, maximizing the ability to remain in a community setting.

Key Responsibilities
  • Identify at-risk patient populations who require care management services, in collaboration with physicians.
  • Serve as a patient/family advocate and act as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
  • Support the mission, values, and management of patient care services, actively incorporating ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
Requirements
  • 2 years of nursing experience or no experience required with preferred education.
  • Diploma in nursing or Associate's degree in nursing required, and Bachelor's degree in nursing preferred.
  • Registered Nurse licensure required.
Work Schedule

Hours vary, with office open hours Monday through Friday, 7:00am to 5:00pm, and Friday, 7:00am to 3:00pm. No weekends or call requirements.
