Key Responsibilities

In this role, you will:
  1. Provide direct patient care, focusing on promoting wellness, managing chronic conditions, and coordinating care with other healthcare providers.
  2. Work closely with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to the residents' needs.
  3. Maintain accurate, timely, and thorough documentation within the electronic medical record (EMR) system, ensuring compliance with regulatory standards and organizational protocols.
Qualifications

To succeed in this role, you must have:
  • A valid nurse practitioner or physician assistant license in state of operations & current board certification in your field.
  • A Master's Degree in Nursing or Physician Assistant Studies.
  • Strong leadership, interpersonal, and problem-solving skills preferred.
