Position Overview:

The Practical Nursing Specialist is responsible for overseeing and delivering healthcare and wellness services (both chronic and acute) to COMHAR's clients with intellectual disabilities in their community residences. The multidisciplinary treatment team includes professionals such as psychiatrists, mental health experts, licensed nurses, peer specialists, and direct care staff. Services are provided both on-site and in natural community environments to enhance learning opportunities.

The Practical Nursing Specialist will monitor and assess clients for health status, dietary needs, and any known or potential medical, health, or psychiatric issues. This role involves investigating, intervening, and communicating as necessary. Providing timely and skilled responses in emergency situations is crucial, as is reviewing and following up on required medical and health-related information for clients. Our aim is to assist individuals in developing skills, enhancing their independence, and stabilizing their psychiatric and physical health symptoms, ultimately facilitating transitions to less structured environments.

COMHAR has immediate openings for both Part-Time and Full-Time Practical Nursing Specialists.

Key Responsibilities:

  • Work within a recovery framework to provide health and wellness care, medication education, group psychoeducation, and therapeutic interventions (preferably evidence-based practices) to help individuals achieve their recovery goals.
  • Oversee the medication administration process for clients, maintaining close communication with pharmacies to ensure accurate medication distribution. Review Medication Administration Records (MARs) for accuracy and completeness, reporting any issues to Site Managers and Assistant Residential Directors.
  • Regularly assess and recommend improvements to unit health and safety procedures and practices, including accident prevention, safety education, emergency protocols, medication administration, and personal care for clients.
  • Assist residents who are unable to self-administer medication, supervise self-administration when appropriate, and administer injections as prescribed. Participate in educating residents about their medications, including supporting those who need assistance in becoming capable of self-administration.
  • Engage in multidisciplinary treatment team meetings as assigned.
  • Contribute to the Quality Improvement Program as assigned.
  • Report all crises and unusual incidents to the Nurse Manager and LTSR Director in a timely manner (immediately if necessary; otherwise within 24 hours). Report psychiatric and medical crises to the Psychiatrist without delay.
  • Ensure compliance with all internal and external regulations regarding record keeping and documentation.
  • Observe Universal Precautions in medication administration, needle disposal, and waste handling.
  • Provide direct services to residents, including assistance with personal hygiene, bathing, and other daily living tasks as needed.
  • Lead activities with residents, including educational, social, and leisure programs, ensuring residents' input is valued. Participate in community integration activities as required.
  • Maintain accurate and relevant documentation in clinical charts and utilize designated forms appropriately.
  • Attend all assigned meetings and training sessions.
  • Report any non-emergent shift and personnel issues to the Nurse Manager and/or LTSR Director promptly and accurately.
  • In the event of staff absences, work to secure appropriate coverage through existing team members or approved temporary staffing organizations.
  • Adhere to all COMHAR policies and procedures without unauthorized exceptions.

Physical Demands:

While performing the duties of this position, the employee is regularly required to communicate effectively and interpret speech and sounds accurately. Frequent standing, walking, climbing, and descending stairs are necessary. The role requires the use of hands and arms to reach, hold, and carry objects, as well as sitting and maintaining balance. Occasional lifting or moving of individuals weighing up to 150 pounds is required. Vision requirements include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust optical focus for reading, driving, and general observation.

Qualifications:

  • Licensed Practical Nurse (LPN) with a minimum of 2 years of mental health experience preferred.
  • Current nursing license is required.
  • Valid driver's license is required.
  • Must have reliable transportation.
  • ACT 31 certification is required.
  • This position may involve occasional lifting and transferring of individuals averaging 150 pounds.

Required Skills:

Excellent verbal and written communication skills are essential, along with the ability to work effectively with people. Strong problem-solving skills, clinical nursing expertise, knowledge of behavioral health systems, and community resources are necessary. The ability to work independently and adapt to changing priorities in a fast-paced environment is crucial. Proficiency in computer skills is required.
