CareMed Health Solutions is a facility-based team of medical providers dedicated to delivering high-quality care to patients in long-term care facilities.

We are seeking a skilled Wound Care Nurse Practitioner to join our team and provide expert wound care services to patients in skilled nursing facilities.

Responsibilities:
  • Provide wound care services for long-term care patients, including diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds.
  • Prescribe appropriate medications, including controlled substances in accordance with KY Law.
  • Order, review, and/or follow-up with treatments and diagnostic testing.
  • Communicate with nursing home staff on a regular basis to discuss changes in condition and/or care plan.
  • Collaborate with primary care physicians to ensure comprehensive patient care.
Requirements:
  • Current, unrestricted NP license in the state of Kentucky.
  • Board Certification.
  • DEA licensure (Optional).
  • Clinical background in adult, family, or geriatric specialties.
  • Clinical background of wound care, including chronic, debilitating, stalled, or infected wounds.
  • Experience working in a long-term care setting.
  • Ability to work with an EMR.
  • Ability to work within an interdisciplinary care team to coordinate patient care.
