Job Summary

We are seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a Nurse Practitioner In-Home Assessment, you will be responsible for conducting comprehensive health risk assessments and providing transitional care to patients discharged from hospitals to prevent re-admissions.

Responsibilities
  • Conduct in-depth health risk assessments to identify active and chronic disease conditions, as well as physical, mental, and social needs.
  • Educate and counsel patients and families based on assessment results, including recommendations for care and treatment.
  • Perform follow-up visits in transitional care to ensure continuity of care.
Qualifications
  • Current RN and NP licensure in the state of practice.
  • Current CPR certification.
  • Enrollment in both Medicare and Medicaid.
