Case Manager

Service Coordination, Inc. is seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing support and services to individuals with Medicaid Home and Community Based services.
Key Responsibilities- Advocacy and Support: Advocate on behalf of individuals served, providing resource information and education to empower them to become more self-determined.
- Person-Centered Planning: Develop and implement Person-Centered Plans of Service (POS) that document service needs for individuals eligible for Supports Planning services.
- Resource Coordination: Research, identify, refer, and coordinate resources and services to achieve the outcomes specified in the individual's POS.
- Monitoring and Evaluation: Monitor POS and conduct follow-up activities to ensure compliance with regulations and SCI standards.
- Communication and Collaboration: Communicate effectively with providers, stakeholders, and team members to ensure seamless service delivery.
- Education: Bachelor's or Master's degree in a human service field, such as Human Services Administration, Psychology, Sociology, Social Work, Nursing, or Counseling.
- Experience: Experience working with individuals with disabilities and person-centered planning is preferred.
- Skills: Strong problem-solving, decision-making, and communication skills, with the ability to work independently and as part of a team.
