About the Role

We are seeking a skilled Nurse Practitioner to join our team at Abode Care Partners. As a primary care provider, you will play a vital role in delivering high-quality medical services to our patients in various settings, including skilled nursing facilities, assisted living, and private homes.

Responsibilities
  • Provide evidence-based medical care to patients with chronic and urgent episodic illnesses
  • Examine and treat patients, ordering and interpreting laboratory and diagnostic testing as needed
  • Collaborate with caregivers and other healthcare professionals to ensure comprehensive care
  • Provide referrals to other healthcare practitioners as necessary
  • Maintain accurate and up-to-date patient medical records
Requirements
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
  • Strong interpersonal communication skills
  • Proficiency in medical instruments and equipment
  • Knowledge of HIPAA and OSHA regulations
  • Proficiency in computer-based data management programs and electronic medical record (EMR) systems
  • Licensed driver with valid automobile insurance
  • Ability to engage in moderate physical activity
