Pharmacist Clinical Specialist

We are seeking a highly skilled Pharmacist Clinical I to join our team at Christus Health. As a key member of our pharmacy team, you will play a vital role in providing exceptional patient care and ensuring the safe and effective use of medications.
Key Responsibilities- Fill orders for drugs, monitor patient drug therapies, and provide drug information to healthcare professionals.
- Prepare and dispense drug orders according to established policies, procedures, and protocols.
- Compound and dispense pharmaceuticals, including non-sterile and sterile products, chemotherapy, and parenteral nutrition products accurately.
- Issue controlled substances to patient care areas and maintain records as required by law and institutional policies.
- Attend staff meetings and supervise and direct pharmacy support personnel.
- Verify the daily activities of pharmacy technicians.
As an Ambulatory Care Pharmacist, you will provide pharmaceutical care services to patients in various clinical settings, including anticoagulation clinics, intensive medical home, transition of care, and discharge medication reconciliation. You will ensure safe, appropriate, and cost-effective drug therapies for patients according to established policies, procedures, and protocols.
You will monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose. You will also read, extract, and interpret information in patient medical records accurately and provide accurate, adequate, and timely drug information to the hospital's professional staff.
You will participate in quality improvement and medication use review activities of the department, collect data, conduct monitors and inspections, and maintain logs, records, and other documentation as assigned.
Please note that you may be exposed to hazardous and toxic substances, including chemotherapeutic, cytotoxic drugs, and cleaning solutions, as well as potential exposure to biologic material when participating in direct patient care activities.
Requirements- Doctor of Pharmacy or BS Pharmacy degree required.
- Pharmacy registration in the State of practice.
- Must have pharmacy State licensure within 60 days of start date.
This is a per diem as needed position.
