OBJECTIVE:

Deliver exceptional patient care under the guidance of a supervising physician, utilizing your training and expertise. Evaluate patient care strategies and provide thorough patient management either independently or in collaboration with other healthcare professionals. Work cohesively with the healthcare team to synchronize patient care and set shared objectives. Use your experience and judgment to strategize and achieve healthcare goals while demonstrating a commitment to patient satisfaction and teamwork.

KEY RESPONSIBILITIES:

  • Gather essential data to facilitate patient evaluations, including vital signs, social history, primary complaints, and current health concerns.
  • Notify the supervising physician of any patient issues, particularly those that may pose immediate risks to life.
  • Assist in clinical tasks to prepare patients for examinations, treatments, or procedures.
  • Manage pharmacy refills and patient follow-ups as per office protocols and physician directives.
  • Document all communications, interactions, and encounters with patients according to established guidelines.
  • Employ the nursing process to assess patients' conditions and educational needs regarding their healthcare, providing guidance on treatment, dietary requirements, medications, follow-up tests, and healthy lifestyle choices.
  • Conduct patient rounds in a dialysis environment according to a predetermined schedule.
  • Administer prescribed medications and treatments as directed by the physician, adhering to nursing standards.
  • Execute procedures and treatments within the scope of training and experience as ordered by the physician, ensuring proper documentation of outcomes in patient records.
  • Manage emergency and non-emergency phone inquiries, directing them to appropriate resources while documenting according to protocols.
  • Follow established protocols for obtaining, preparing, and transporting specimens for diagnostic testing as instructed by the physician, employing universal precautions.
  • Monitor, document, and report on patients' conditions and responses to medications and treatments to the supervising physician.
  • Assist in coordinating patient transfers to hospitals for direct admissions.
  • Analyze and interpret various professional documents, including business periodicals, journals, technical procedures, and regulatory guidelines. Prepare reports, business correspondence, and procedural manuals. Present information and address inquiries from groups of managers, clients, and the public.
  • Adhere to established protocols for reporting and documenting diagnostic tests as directed by the physician, within the limits of training and experience.
  • Review patient records and relevant information, taking appropriate actions in line with nursing standards and office protocols.
  • Administer medications as prescribed by the physician and ensure accurate documentation in the patient's medical record.
  • Document and relay clinical information or patient complaints received via phone to the physician in a timely and appropriate manner.
  • Address diverse problems where data analysis requires evaluating identifiable factors, demonstrating sound judgment in selecting methods and techniques for resolution.
  • Escalate issues to supervisors or managers for resolution when necessary.
  • Mentor and guide other staff members as applicable.
  • Comply with the Code of Business Conduct and all relevant company policies, local, state, and federal regulations.
  • Assist with various projects as assigned by direct supervisors.
  • Perform other duties as required.

  • Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Travel may be required.

EDUCATION:

  • Master's Degree is required.
  • Completion of university graduate-level education, nationally Board Certified in the specialty area based on NP licensure and state authority for advanced practice.
  • Nurse Practitioner License is mandatory.
  • CPR Certification is required.

EXPERIENCE AND REQUIRED SKILLS:

  • 1 - 3 years of experience or 1 year of directly related experience in Nephrology.
  • Excellent verbal and written communication abilities.
  • Familiarity with software applications commonly used in medical offices, including electronic medical records, scheduling systems, and standard office equipment.
  • Ability to perform calculations involving discounts, interest, commissions, proportions, percentages, area, circumference, and volume, as well as apply basic algebra and geometry concepts.
  • Capable of solving practical problems and addressing a variety of concrete variables in situations with limited standardization, interpreting diverse instructions provided in written, oral, diagram, or schedule form.
