We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team at the Karen Ann Quinlan Memorial Foundation. As a Palliative Care Nurse Practitioner, you will play a critical role in providing high-quality patient care and support to individuals and their families.
Key Responsibilities:- Provide and document medically necessary services in accordance with physician/NP protocol.
- Develop and implement treatment plans that maximize patient health potential.
- Consult with collaborative physicians, consulting practitioners, primary care physicians, and other specialist physicians.
- Establish relationships with hospital-based palliative care teams, partner organizations, medical directors, hospital physicians, community physicians, nurses, and other team members.
- Refer patients to medical specialists and/or other healthcare services as medically indicated.
- Assist in all facets of care coordination for referrals.
- Develop outreach programs for community agencies and healthcare settings.
- Provide disease management instruction and education to patients and their families.
- Provide clinical guidance to facility staff relative to patient care issues, assessments, and interventions.
- Perform on-call services as applicable.
- Participate in interdisciplinary team meetings as appropriate.
- Prepare and maintain accurate patient records, charts, and documents.
- Act as a clinical resource to coordinate complex cases for safe and appropriate transitions to other care settings.
- Comply with applicable laws and regulations regarding delegation protocol agreements, prescriptive authority, and NP scope of practice.
- Review policies and services with referred patients and/or family caregivers or authorized patient representatives and obtain consent for medical care.
- Work collaboratively with the Intake department to verify coverage/payment and initiate reimbursement for services rendered.
- Assist in the development and review of clinical practice guidelines/standards in support of quality care.
- Act with integrity, compassion, and customer focus to ensure quality care.
- Adhere to employment policies as described in the employee handbook and maintain confidentiality.
- Participate in all required in-service education programs and meetings.
- Demonstrate compliance with the dress code policy by appearing well-groomed, wearing an ID badge while on duty, and maintaining a professional appearance.
- Knowledge of the palliative care philosophy of care and the delivery of medical care at home.
- Knowledge of the dying process and policies and procedures of the hospice organization.
- Maintains a broad base of technical knowledge and skills to perform all assigned clinical/administrative duties.
- Ability to understand and follow state regulations regarding practice agreement with physician.
- Ability to function as a resource to patients and families during times of high stress and volatility.
- Ability to assess patients to determine appropriateness of participation in hospice.
- Ability to build constructive relationships with colleagues, and outside agencies, organizations, and individuals.
- Excellent communication skills to effectively deliver information between referral source and the hospice clinical team.
- Consistently model respect for, and accountability to, the public, patients/families, and co-workers.
- Conveys a professional and positive image and attitude regarding the hospice organization.
Education and Experience:
Required Education: Board-licensed nurse practitioner, licensed to practice in New Jersey (PA license may also be required depending on territory served). Hospice and Palliative Care Certification. Complies with current department of Public Safety and maintain Drug Enforcement Agency registrations. DEA Licensure required.
Preferred Education:
Required Experience: Experienced in the practice of family practice, oncology, or hospital practice.
Preferred Experience: Prior experience as a nurse practitioner.
