About the Job
">We are seeking a dedicated Palliative Care Nurse Specialist to join our team at Chapters Health System. As a Palliative Care Nurse Specialist, you will play a vital role in providing high-quality patient care and support to individuals with serious illnesses.
">Job Responsibilities:

- Provide direct care to patients and families, incorporating psychosocial, spiritual, cultural, physical, and biological components into their care plans.">
- Coordinate patient care, ensuring that individualized plans of care accurately reflect the evolving needs of patients.">
- Educate patients, families, caregivers, and healthcare professionals about disease processes, palliative interventions, caregiving, dying processes, and safety practices.">
- Participate in Interdisciplinary Group (IDG) evaluations of hospice referrals and admissions to ensure level of care appropriateness.">
- Attend patient deaths, prepare bodies for transport, and oversee medication disposal.">
- Provide bereavement interventions to families and caregivers in conjunction with psychosocial staff.">
- Present concise and pertinent verbal and written reports to IDG members and respect input from all disciplines.">
- Communicate effectively with physicians, staff members, patients, families, and supervisors, utilizing positive approaches when working with others.">
Requirements:
">- ">
- Current Florida license as an RN, active single-state out-of-state nurse license, or current active multistate/compact nurse license.">
- Minimum one year of in-patient nursing or hospice experience for external candidates; minimum of six months of LPN experience with hospice/home health within an affiliate for internal applicants.">
- Previous experience working with Electronic Medical/Health Record systems.">
- Ability to use equipment with visual and auditory mechanisms.">
- Ability to communicate effectively in English (verbal and written).">
- Flexible schedule to meet the needs of Hospice House patients.">
- Ability to manage the emotional stress of working with terminally ill patients and their families.">
- Ability to physically access patient rooms and other areas of the Hospice House.">
- Ability to perform essential functions and physical requirements of the job, including lifting patients and/or equipment, bending, pushing/pulling, kneeling.">
- Active BLS for healthcare professionals from the American Heart Association or Red Cross.">
- Intermittent driver - valid driver's license and automobile insurance per Company policy.">

Physical Demands:
">This position requires frequent standing, sitting, reaching with hands/arms, lifting, bending, balance. Occasional pulling, pushing, stooping/crouching, kneeling, climbing stairs.
">Estimated Salary: $70,000 - $90,000 per year, depending on experience.
