Overview:
The Lead Therapist plays a crucial role within a multidisciplinary team, dedicated to delivering direct clinical services to clients and their families. This position emphasizes the completion of comprehensive Psychosocial Assessments, as well as providing individual, group, and family therapy. Responsibilities also include facilitating psycho-educational training, conducting family meetings, and assisting in discharge planning.
Key Responsibilities:- Conduct psychosocial assessments and formulate initial discharge plans.
- Design and execute treatment plans tailored to client assessments, coordinating additional services as required.
- Facilitate individual, group, or family therapy sessions as dictated by client treatment plans.
- Utilize psychotherapeutic techniques and interventions to address diagnosed emotional and behavioral disorders.
- Engage in multidisciplinary treatment planning conferences, presenting relevant client information and recommending effective treatment strategies.
- Manage an assigned caseload effectively.
- Develop and implement specialized client groups or program modules.
- Prepare timely progress notes and treatment updates, ensuring compliance with clinical documentation standards.
- Ensure each client has a structured and clinically appropriate post-care plan, acting as a liaison to referral sources to support continuity of care.
- Provide crisis intervention for clients and families experiencing acute distress.
- Coordinate a wide range of community resources, including financial, housing, legal, social, recreational, occupational, spiritual, and support services.
- May engage in case management, discharge, or aftercare planning as necessary.
- Bachelor's Degree in Social Work, Counseling, or Psychology is required.
- A minimum of one year of experience as a therapist working with diverse age groups is essential.
