Job Summary:The Public Health Nurse I will provide assessments of individuals, families, and communities, and develop treatment plans to address their health needs. You will work closely with other healthcare professionals to coordinate care and provide consultation on medical, dental, and behavioral health assessments.
Responsibilities:- Provide assessments of individuals, families, and communities to identify health needs and develop treatment plans.
- Work closely with other healthcare professionals to coordinate care and provide consultation on medical, dental, and behavioral health assessments.
- Develop and implement disease control measures to prevent the spread of illness and disease.
- Act as a liaison with public and private agencies to provide assessments and referrals for school district students.
- Conduct health education and promotion activities to promote healthy behaviors and prevent disease.
- Valid Registered Nurse (RN) license issued by the California Board of Registered Nursing (CA BRN).
- Valid California Public Health Nurse certification issued by the CA BRN.
- Valid Healthcare Provider level Basic Life Support (BLS) certificate.
- Bachelor's degree in Nursing (BSN) or higher.
- Experience working with pediatric populations, including enhanced care management experience, certificate or training, and the ability to use social determinants of health in developing care plans and interventions.
- Fluency in American Sign Language, Arabic, Korean, Mandarin, Spanish, and/or Vietnamese.
- Computer skills in Microsoft applications, including Microsoft Word, Excel, and Outlook.
