- Monitor residents experiencing weight fluctuations, enteral feeding needs, dialysis, and wound care.
- Engage in resident review meetings to discuss and strategize care for high-risk individuals.
- Perform patient nutrition assessments across a diverse population with varying health complexities.
- Develop and implement nutrition intervention plans in alignment with the Nutrition Care Process.
- Provide personalized nutrition counseling, adapting teaching methods to accommodate individual learning needs.
- Utilize assessment techniques that respect the unique needs of different age groups and cultural backgrounds.
- Refer patients to outpatient counseling or community services as necessary.
- Work collaboratively with medical and food service teams to ensure adherence to prescribed nutrition care protocols.
- Deliver educational sessions that promote compliance with food safety and sanitation standards.
- Maintain accurate and timely documentation of the nutrition care plan within patient medical records, detailing assessments, diagnoses, plans, implementations, and progress towards health goals.
Qualifications:
- Bachelor's degree in Dietetics or a related field is required; a Master's degree is preferred.
