- Provide counseling and support to patients and their families, addressing issues related to diagnosis, treatment, and coping mechanisms
- Collaborate with healthcare teams to ensure seamless care transitions and coordinate community resources
- Develop and implement advance care plans, addressing patient and family needs
- Supervise social work interns and provide guidance on best practices
- Participate in daily rounds and contribute to patient care planning
- Master's degree in Social Work from an accredited institution
- Minimum 1 year of experience in a healthcare setting, with a preference for 2+ years in an acute care setting
- Knowledge of medical terminology and ability to communicate effectively with patients and staff
- Strong organizational and time management skills, with ability to prioritize tasks and manage multiple cases
- Ability to work in a fast-paced environment and maintain confidentiality
