Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Advantmed provider network. As a Risk Adjustment Specialist, you will conduct in-home wellness risk adjustment assessments for Medicare and other populations, ensuring accurate and comprehensive risk adjustment coding and promoting value-based care.

Responsibilities
  • Perform annual wellness visits and health assessments on populations with chronic conditions.
  • Deliver patient health education opportunities to promote informed decision-making.
  • Assist in closing quality care gaps, such as screenings and labs.
  • Collaborate with our team to enhance care delivery and patient outcomes.
Requirements
  • Valid unencumbered NP License for the state of employment.
  • Previous in-home Risk Assessment experience preferred.
  • 3 years patient care experience preferred (primary care/adult/geriatric, EMR).
  • Bilingual is a plus.
