Position Overview
As an Advanced Practice Provider with Summit Health, you will collaborate with a multidisciplinary care team to deliver exceptional primary care.
Outstanding Patient Care:
- Delivering personalized, professional care directly to patients
- Ensuring accurate and up-to-date patient documentation
Clinical Expertise:
- Assessing patients' current health issues, establishing diagnoses for effective care management
- Conducting physical assessments, medication evaluations, wound care, and mental health screenings
Collaborative Practice:
- Partnering with other healthcare providers, APPs, and physicians to offer superior care in a fully outpatient primary care setting
- Communicating findings to the primary care provider to address potential care gaps
- Graduation from an accredited advanced practice program with certification
- A minimum of two years of outpatient clinical experience as an Advanced Practice Provider, demonstrating the ability to work independently
- Proficient in treating adult patients and conducting comprehensive health assessments
- Possession of a current, unrestricted state license or eligibility for licensure
- Active DEA registration or eligibility for registration
- Current Medicare/Medicaid enrollment or eligibility to enroll
- Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Strong verbal and written communication skills
- Familiarity with Electronic Medical Record (EMR) systems; experience with Athena preferred
- Proficient in Microsoft Office applications
