Key Responsibilities

• Collaborate with the Mind Wellness Team to provide social work case management services
• Serve as an advocate for patients and families to navigate care
• Use collaborative problem-solving skills to meet the needs of patients and providers
• Provide social work case management services as needed
• Rely on resources such as family, community, state, and federal agencies to assist patients and families

Requirements

• Bachelor's degree in Social Work or a related field
• Social work licensure in the state of WV (LSW required)
• Minimum of 1 year of position-related experience
• 2-5 years of position-related experience preferred
