About the Role

We are seeking a skilled Physical Therapist Assistant (PTA) to join our Home Health team in South Portland, Maine. As a PTA, you will play a vital role in providing personalized therapy to patients in their homes, helping them regain strength, manage pain, and achieve their goals.

As a member of our Rehabilitation Team, you will work collaboratively with a multidisciplinary team to develop and implement patient care plans. Your expertise will be invaluable in helping patients overcome mobility challenges and improve their overall quality of life.

Responsibilities
  • Provide home-based physical therapy to patients with mobility, strength, and functionality challenges
  • Work collaboratively with the home health team to develop and implement patient care plans
  • Conduct assessments, document progress, and modify care plans as needed
  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients on their plan of care, established goals, exercises, and self-care
  • Coach family members on the plan of care, therapy goals, and safely assisting with exercises
Requirements
  • Active PTA license in Maine
  • Minimum of 1 year of PTA experience, ideally in home healthcare
  • CPR certification
  • Up-to-date on all State of Maine Required vaccinations
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion
