As a key member of our interdisciplinary care team, you will provide patient-centered care and support to patients and their families in the comfort of their own homes.
Responsibilities- Assist patients and families with health-related financial, social, and emotional concerns, and provide effective coping skills.
- Provide social work services in accordance with the plan of care, ensuring timely communication of changes in patient status to healthcare team members and care center office staff.
- Educate patients, family members, and other team members on medical social work interventions and treatment plans.
- Perform care planning, transfer, and discharge functions, identifying and mitigating risks to patient health in their environment.
- Communicate risk identification to physicians and other healthcare team members to coordinate effective care.
- Holds a master's or doctorate degree from a school of social work, accredited by the Council on Social Work Education.
- Has one year of social work experience in a healthcare setting.
- Meets state and federal requirements for this position.
- Current CPR certification.
