Position Overview
The Clinical RN Case Manager operates under the guidance of the Nurse Manager and Director, playing a crucial role in the assessment, planning, implementation, and evaluation of comprehensive nursing care for assigned patients on an intermittent visiting basis. This position is integral to Riverside Health System, where the RN may engage in various settings, primarily focusing on home care. Utilizing strong communication skills, critical thinking, and problem-solving abilities, the RN develops tailored care plans that consider the physiological, psychosocial, and cultural diversity of each patient. The RN is also responsible for supervising and delegating tasks to non-professional staff to ensure optimal patient care.

Key Responsibilities

  • Conducts thorough assessments, plans, implements, and evaluates nursing care for patients on the caseload, developing both initial and ongoing comprehensive evaluations of client needs, including the effects of terminal diagnoses on their physical and psychological well-being.
  • Evaluates patients' risks for complicated grief, cultural and spiritual considerations, and communication styles. Assesses caregivers' abilities to meet patients' immediate needs upon admission and throughout the care process.
  • Ensures accurate and timely completion of all clinical documentation in accordance with Riverside Health System standards, including patient notes, verbal orders, and necessary certifications.
  • Initiates and manages individualized care plans, adjusting them as necessary to address patient discomfort and provide symptom relief, while also educating patients and families on self-care techniques and end-of-life considerations.
  • Employs a case management approach to coordinate patient services, prioritizing needs in collaboration with interdisciplinary team members and ensuring ongoing evaluation of patient responses to care.
  • Supervises LPNs and Home Health Aides, providing specialized training and support to ensure quality care delivery. Participates in the training and development of team members, volunteers, and students as required.
Qualifications

Education
  • Registered Nurse (Required)
  • Bachelor's Degree in Nursing (Preferred)
Experience
  • Experience as a Staff Nurse (Preferred)
  • Experience in Home Care (Preferred)
Licenses and Certifications
  • Registered Nurse (RN) - State Department of Health Professions (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days (Required)
  • Valid Driver's License (Required)
