Key Responsibilities:
• Conduct thorough assessments of patients to develop individualized care plans
• Coordinate communication with families, nursing home staff, social workers, and other team members
• Complete initial plans of care, including medication lists and physician orders
• Verify orders with physicians and ensure accurate documentation
• Offer and administer immunizations as needed
Requirements:
• Graduate of an accredited nursing program
• Current nursing license in good standing
• Minimum two years of nursing experience, preferably in a post-acute setting
• Excellent communication and documentation skills
• Ability to lift, transfer, and move patients with weights up to 50 pounds
