Job Summary
The successful candidate will be responsible for providing high-quality patient care in a long-term care setting. This will involve assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities
- Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions
- Assess patient needs and develop individualized care plans to meet those needs
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure optimal patient outcomes
- Participate in patient education and discharge planning to ensure a smooth transition to home or other care settings
Requirements
- Current RN licensure in the state of Pennsylvania
- At least 1 year of experience in a long-term care setting
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
