Job Summary

We are seeking a dedicated and compassionate Physical Therapist Assistant to join our team of skilled home health care providers. The ideal candidate will have a strong commitment to providing quality care to our patients in their homes.

Responsibilities
  1. Assist in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
  2. Treat clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light and electricity.
  3. Teach, supervise, and counsel the family and client in the physical therapy program.
  4. Instruct client in care and use of adaptive devices such as wheelchairs, braces, crutches, canes and prosthetic and orthotic devices.
  5. Arrange for out-patient service for procedures when appropriate.
  6. Record on a timely basis all treatments, and client's response to therapy intervention Also records quality assurance data.
  7. Communicate with PT and other team members (including Home Health Aides), when appropriate. Instruct them in certain phases of physical therapy that they may use while working with the client
  8. Under the direction of a Physical Therapist, write clinical/progress notes on the date of the visit and incorporate same in the client's record according to company policy.
  9. Participate in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary.
  10. Participate in effective coordination of schedule to ensure all reassessments are performed in a timely manner.
  11. Obtain a medical history, particularly as it relates to the present condition.
  12. Under the supervision of the physical therapist, assist in evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
  13. As directed, perform routine strengthening and therapeutic exercises and assist the patient with the use of self-help devices.
  14. Identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and report same to the PT.
  15. Observe, and record activities and findings in the clinical record and report to the PT and the physician any adverse reaction to treatment.
  16. Participate with staff, clients, and physicians in discharge activities and assist the Physical Therapist in completing the Physical Therapy Discharge Summary.
  17. Assume other duties deemed appropriate and necessary by the PT and as designated by Supervisor
Requirements
  • The ability to drive and the ability to access clients' homes, which may not be routinely wheelchair accessible are required.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
  • May be required to bend, stoop, reach and move client weight up to 250 pounds, lift and/or carry up to 30 pounds.
  • Must be a graduate of an approved school giving a course of not less than two years for Physical Therapist Assistants.
  • Must be licensed by the State as a Physical Therapist Assistant.
  • Must have one year of experience in an acute/sub-acute setting.
