Nurse Practitioner

Abode Care Partners is seeking a compassionate and skilled Nurse Practitioner to join our team of healthcare professionals. As a Nurse Practitioner with our company, you will have the opportunity to provide high-quality medical care to seniors and individuals with complex needs in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes.
Responsibilities- Provide Evidence-Based Medical Care: Plan, direct, and evaluate patient medical needs, ensuring the highest level of care and quality of life.
- Examine and Treat Patients: Conduct thorough examinations and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
- Order and Interpret Laboratory and Diagnostic Testing: Order and interpret laboratory and diagnostic testing as needed to inform patient care and treatment plans.
- Collaborate with Healthcare Professionals: Work collaboratively with caregivers and other healthcare professionals to provide interdisciplinary care and maintain a comprehensive plan of care.
- Provide Referrals: Refer patients to other healthcare practitioners when necessary to ensure comprehensive care.
- Ensure Accurate Medical Records: Maintain accurate and up-to-date patient medical records.
- Active State License: Possess an active state Nurse Practitioners license.
- Current Certification: Hold current Nurse Practitioner National Board Certification.
- Active DEA Licensure: Possess active state DEA licensure.
- Master's Degree in Nursing: Hold a Master's degree in Nursing (MSN).
- Experience and Skills: Possess five or more years of nursing experience, with at least two years of nurse practitioner experience preferred. Demonstrate strong interpersonal communication skills, excellent time management skills, and proficiency in medical instruments and equipment.
- Knowledge and Compliance: Possess a strong knowledge base of HIPAA and OSHA regulations and demonstrate proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
- Physical Demands: Be able to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day, etc.
