The Mount Sinai Health System is seeking a highly skilled Healthcare Services Coordinator to play a crucial role in the delivery of healthcare services for patients within the assigned area. As a key member of our team, you will serve as a liaison for patient care activities in specific programs and ensure coordination of care around the course of the patient visit as well as subsequent patient interactions between visits.
Responsibilities- Lead Patient Navigation: Play a lead role in the navigation of care and ensure that patients understand their overall program benefits as well as what they can expect the program to provide. This may include liaison with physicians, patient education, and coordination of care.
- Goal Setting and Tracking: Set goals with patients and care teams to coordinate care, document goals in program dashboards, and track follow-ups, including visit status reminders, scheduled follow-up appointments, and authorization submissions.
- Order Authorization: Ensure that all orders placed by providers (internal and external) are authorized after confirming that the service falls under the patient's covered services.
- Pharmacy Benefits and Prior Authorizations: Serve as the primary point of contact for patient education on pharmacy benefits and assist with obtaining prior authorizations when applicable, including submitting refill requests, verifying coverage, and confirming patient preferred pharmacies.
- Correspondence and Program Planning: Organize correspondence and future program plans for patients based on program interactions in the electronic medical record and program benefits systems, ensuring that a plan is made with next steps in the process.
- Patient Navigation and Education: Offer extensive patient navigation between program visits and other encounters, including detailed discussions with patients to review next steps and resources, facilitation of patient concerns review, and questions or concerns related to coverage.
- Patient Education and Communication: Educate and communicate closely with patients to ensure they have a full understanding of the benefits offered by the program, including patient education on all relevant program benefits.
- Process Improvement and Engagement: Provide recommendations for process improvement and services improvement aimed at improving patient experience and participate in ongoing pilot programs to improve patient engagement.
