Job Overview

We are seeking a skilled Care Coordinator Specialist to join our team. As a key member of our staff, you will play a vital role in ensuring seamless communication between external inpatient/hospital staff and our employees.

This full-time position offers a competitive salary and benefits package, with a estimated annual salary of $65,000 - $80,000 depending on experience.

Responsibilities
  • Coordinating care for clients in an inpatient setting, ensuring timely scheduling of post-discharge appointments.
  • Collaborating with inpatient and outpatient providers to facilitate effective communication and discharge planning.
  • Notifying relevant authorities and agencies of risk situations, such as Adult Protective Services and law enforcement.
  • Demonstrating adaptability and prioritization skills to manage frequent interruptions and tasks.
  • Providing clear oral and written communication while performing essential functions.
Requirements
  • Masters' Degree in Counseling or Social Work required; current licensure from the Arizona Board of Behavioral Health Examiners preferred.
  • Three years' experience in discharge planning, care coordination, or crisis counseling.
  • Experience working in a multicultural healthcare environment, with direct patient serving experience preferred.
  • Valid Arizona drivers' license with no major violations in the past five years.
  • Able to relate to diverse cultures and communities.
  • Ability to obtain an AZ Department of Public Safety Fingerprint Card within 60 days.
