Position Summary:
The Pharmacist, operating under the general oversight of the Pharmacy Director, is responsible for reviewing medication orders by utilizing physician directives to establish and maintain comprehensive patient medication records. This role involves dispensing pharmaceuticals through a unit-dose/IV admixture system, providing clinical interventions in patient care, supervising pharmacy technicians and students, and engaging in departmental educational initiatives.
Qualifications:
Educational Background:
A degree in pharmacy (Bachelor of Science, Doctor of Pharmacy, or Pharm. D.) is required.
Essential Skills:
Proficiency in intravenous admixture techniques is essential, along with a moderate level of competency in using a keyboard, electronic calculator, and electric typewriter (typing speed of 20-30 wpm).
Licensure Requirements:
Must hold a valid Registered Pharmacist license from the Louisiana State Board of Pharmacy and current BLS certification.
Key Responsibilities:
1. Uphold and safeguard patient confidentiality in all aspects of care, adhering to the hospital's ethical standards and patient rights.
2. Exhibit compassionate behaviors that enhance satisfaction for patients, families, staff, and physicians (courtesy, kindness, helpfulness, empathy).
3. Evaluate drug therapy based on:
- Therapeutic appropriateness considering age, weight, diagnosis, and current medical status.
- Potential interactions, incompatibilities, and duplications.
- Patient safety and cost-effective treatment options.
4. Receive, prioritize, interpret, and process medication/IV orders via computer systems, exercising independent judgment to identify issues that require referral to the Pharmacy Director.
5. Accurately dispense medications, ensuring correct strength, dosage form, and addressing any infrequent medication errors through proper reporting procedures.
6. Document the completion of order reviews by filing comprehensive orders under user-specific logins/passwords into scanning software consistently.
8. Prepare intravenous admixtures and other sterile products using aseptic techniques under a laminar flow hood, maintaining compliance with all relevant guidelines.
10. Provide education to patients and family members regarding prescribed medications, including precautions and other relevant information, utilizing appropriate resources to address inquiries from medical staff and patients.
