Key Responsibilities- Provide patient-centered care to individuals with serious illnesses, focusing on pain management, symptom control, and emotional support.
- Collaborate with interdisciplinary teams, including physicians, social workers, and chaplains, to develop and implement comprehensive care plans.
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs, and document findings in electronic health records.
- Develop and implement individualized plans of care, including medication management, wound care, and palliative interventions.
- Provide education and support to patients, families, and caregivers on end-of-life care, pain management, and advance care planning.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current, unrestricted Registered Nursing License in the state of employment.
- One year of experience in hospice or palliative care, or a related field.
- Valid driver's license, auto insurance, and access to a reliable vehicle.
- Ability to develop and maintain knowledge of hospice and end-of-life care principles.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
