We are seeking a skilled LPN Charge Nurse to join our team at Pocahontas Nursing & Rehab. As a key member of our nursing staff, you will be responsible for providing high-quality patient care and leading our team of LPNs in a long-term care setting.

Key Responsibilities
  • Communicate effectively with doctors, families, and other healthcare professionals to ensure seamless patient care.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
  • Administer medications and process orders in a timely and efficient manner.
  • Collaborate with the healthcare team to develop and implement individualized patient care plans.
  • Supervise and mentor LPNs, providing guidance and support to ensure excellent patient outcomes.
Requirements
  • Current LPN licensure in the state of practice.
  • Experience in long-term care or geriatric nursing.
  • Strong leadership and communication skills.
  • Ability to work effectively in a fast-paced environment.
