The Nurse Case Manager IHR Delnor is a key member of our healthcare team, responsible for promoting continuity of care, optimal patient outcomes, and patient satisfaction. This role requires strong clinical expertise, effective communication skills, and a commitment to delivering high-quality patient care.
Key Responsibilities- Monitor and manage patient care to ensure continuity and optimal outcomes.
- Collaborate with nursing staff and multidisciplinary teams to evaluate patient status and appropriateness of medical care.
- Monitor patient and family satisfaction, responding to concerns and complaints in a timely and professional manner.
- Participate in discharge planning, coordinating patient transfers and community resources as needed.
- Provide education and resource referrals to patients, promoting self-management and independence.
- Perform chart reviews to identify potential issues with service delivery, patient outcomes, and satisfaction.
