RN Home Health Clinical Manager

We are seeking a highly skilled and experienced RN Clinical Manager to join our team at Always Best Care Senior Services - San Diego, CA. As a key member of our leadership team, you will be responsible for guiding and leading our home health clinical staff to deliver high-quality patient care.
Key Responsibilities:
- Develop and implement nursing services, objectives, policies, and procedures in accordance with Federal and State regulations.
- Coordinate, organize, implement, and evaluate the patient care delivery system to ensure timely and accurate completion of physician orders.
- Monitor clinical documentation for completeness and accuracy, and provide education to clinical staff and patients/families as needed.
- Collaborate with physicians, social workers, speech therapists, physical therapists, occupational therapists, and other clinical staff to monitor patient status, effectiveness of patient care plans, and appropriateness for home health care.
- Identify issues and direct changes to ensure efficient, cost-effective, and appropriate care delivery.
- Collect and analyze data to inform decision-making and drive continuous improvement.
- Ensure compliance with regulatory requirements, including ACHC standards and reimbursement criteria for third-party and private payers.
Requirements:
- RN with current license to practice in California.
- Minimum of five years of nursing experience, with a preference for at least one year in home care.
- Knowledge of community resources, Medicare and state regulations, ACHC requirements, and reimbursement criteria for third-party and private payers.
- Excellent communication, interpersonal, and documentation skills.
- Demonstrated organizational and time management skills, critical thinking, and ability to work independently.
- Computer data entry skills.
Ideal Candidate:
We are looking for a seasoned RN Clinical Manager with at least 3-5 years of experience in a Medicare Home Health environment and a current RN license in California. The ideal candidate will have a proven track record of leading and managing teams, with at least 2-3 years in a leadership role, and a strong desire to provide exceptional customer service and creative clinical management.
