We are seeking a highly skilled Licensed Practical Nurse to join our team at Harrison Senior Living of Chester County. As a key member of our healthcare team, you will be responsible for providing high-quality resident care and ensuring the delivery of exceptional patient experiences.
Key Responsibilities- Assume charge and direction of the nursing unit, collaborating with the Director of Resident Services to ensure seamless care delivery.
- Develop and implement nurse aide assignments to meet resident needs, ensuring timely and effective care.
- Communicate effectively with residents, families, and healthcare teams to address concerns and provide updates.
- Administer medications and treatments in accordance with established policies and procedures.
- Conduct daily assessments and update resident support plans as needed.
- Assist with resident activities of daily living, meal preparation, and fluid intake to promote optimal nutrition and hydration.
- Accompany physicians on resident visits to ensure continuity of care and provide necessary information.
- Transcribe physician orders in a timely manner and maintain accurate records.
- Monitor nursing care and visit residents throughout the shift to ensure adherence to care plans.
- Participate in receiving and giving report to ensure seamless care transitions.
- Complete incident/accident reports and maintain confidentiality.
- Collect resident samples for testing as needed and communicate results to healthcare teams.
- Ensure a clean, safe, and orderly environment for residents, staff, and visitors.
- Document all care provided to residents during the shift and before the end of the shift.
- Graduated from an accredited school of nursing and possess a current Practical Nurse license in good standing from the Commonwealth of Pennsylvania.
- Demonstrated management, communication, and observation skills with a focus on geriatric care.
- Working knowledge of computers and Microsoft Word required.
