Job Summary

The ideal candidate will have a strong understanding of the services and systems available in the community, as well as experience working with individuals with serious mental illness. You will be responsible for maintaining relationships with landlords, completing reviews and assessments, and coordinating services and communication with public and private providers.

This is a full-time position that requires a valid driver's license and the ability to work independently. You will be part of a team that is dedicated to providing high-quality services to individuals in need.

Key Responsibilities:

  • Maintain relationships with landlords and complete reviews and assessments
  • Coordinate services and communication with public and private providers
  • Work directly with program participants to ensure permanent supportive housing is secured
  • Complete interim and annual reviews, HQS in sections, initial needs assessments, initial intakes, and annual assessments

Requirements:

  • High School Diploma or relevant education and experience
  • One year of relevant mental health, developmental disability or substance abuse work experience
  • Knowledge of the services and systems available in the community
  • Ability to work independently and as part of a team
