We are seeking a compassionate and skilled Care Coordinator to join our team at Amorem. As a Care Coordinator, you will play a vital role in providing high-quality, patient-centered care to individuals and their families in the comfort of their own homes.

Key Responsibilities
  • Evaluate Patient Needs

Assess the physical, emotional, and social needs of patients and develop personalized care plans to address their unique requirements.

Coordinate Care Services

Work closely with healthcare professionals, family members, and caregivers to ensure seamless coordination of care services, including medication management, education, and support.

Provide Emotional Support

Offer emotional support and guidance to patients and their families, helping them navigate the complexities of the healthcare system and make informed decisions about their care.

Communicate Effectively

Communicate clearly and effectively with patients, families, and healthcare professionals to ensure that all parties are informed and engaged in the care process.

Monitor Progress

Regularly monitor patient progress, adjusting care plans as needed to ensure optimal outcomes and quality of life.

Collaborate with Interdisciplinary Team

Work collaboratively with the interdisciplinary team, including physicians, nurses, social workers, and other healthcare professionals, to ensure comprehensive and coordinated care.

Document Patient Information

Accurately document patient information, including medical history, treatment plans, and progress notes, to ensure continuity of care and compliance with regulatory requirements.

Requirements
  • Education and Experience

A bachelor's degree in nursing or a related field, with a minimum of 2 years of experience in home healthcare or a related field.

Skills and Qualifications

Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.

Certifications and Licensure

Current licensure as a registered nurse (RN) in the state of practice, with certification in home healthcare or a related field.
