Gerontology Nurse Practitioner Position in Senior Care

Abode Care Partners is seeking a skilled Adult - Gerontology Nurse Practitioner to provide evidence-based medical care to seniors and patients with complex needs in various settings.
This role involves examining, treating, and managing chronic illnesses, as well as collaborating with interdisciplinary healthcare teams to ensure comprehensive patient care.
Responsibilities- Performing routine medical exams and providing treatments for chronic and urgent episodic illnesses, minor injuries, and accidents
- Ordering and interpreting laboratory and diagnostic testing as needed
- Collaborating with caregivers and other healthcare professionals to maintain a comprehensive plan of care
- Ensuring patient medical records are up-to-date and accurate
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- At least five years of nursing experience, with two years of nurse practitioner experience preferred
- Strong interpersonal communication skills, both verbal and written
- Excellent time management skills and accountability for daily workflow
- Proficiency in medical instruments and equipment
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
- Licensed driver with valid automobile insurance
- Able to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day
