**Medication Care Manager LPN Role Overview**



As a Medication Care Manager LPN at Sunrise Senior Living, your role will be to deliver high-quality care and services to our residents. Your primary responsibility will be to administer medications and treatments in a safe and organized manner.



Responsibilities

Medication Administration

  • Administer medications and treatments to residents as prescribed by their healthcare providers.
  • Maintain accurate records of medication administration, including documentation of any changes or issues that may arise.
  • Communicate effectively with residents, families, and healthcare providers to ensure that medication needs are met.
  • Conduct medication reviews and updates as necessary to ensure that residents are receiving the most appropriate medications for their needs.


Resident Care

  • Provide individualized care to residents, including assistance with activities of daily living (ADLs) and ensuring that their physical and emotional needs are met.
  • Conduct service and health updates as directed by the Resident Care Director (RCD).
  • Participate in the development and implementation of individualized service plans (ISPs) to ensure that residents receive the care and services they need.
  • Document resident progress and outcomes in accordance with Sunrise Senior Living's policies and procedures.


Risk Management and General Safety

  • Ensure that Sunrise Senior Living's policies and procedures related to medication administration and resident care are followed at all times.
  • Identify and report any potential safety risks or hazards to the RCD or Wellness Nurse.
  • Participate in training and education programs to stay up-to-date on the latest best practices in medication administration and resident care.
  • Collaborate with other members of the care team to ensure that residents receive comprehensive care and services.


Requirements


  • Current LPN licensure in good standing.
  • Experience working in a long-term care or assisted living setting.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Ability to prioritize tasks and manage time effectively.
  • Ability to maintain accurate records and documentation.
  • Ability to follow Sunrise Senior Living's policies and procedures.
