Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Mass General Brigham in West Hatfield, Massachusetts.
About the Role:As a Physical Therapist, you will provide high-quality physical therapy services to patients in an outpatient setting. You will work collaboratively with an interdisciplinary team to assess, evaluate, and treat patients with impairments, functional limitations, or disabilities resulting from injury, disease, or other causes.
Responsibilities:- Assess and evaluate patients' physical therapy needs according to organizational and professional guidelines.
- Develop and implement individualized treatment plans that reflect patients' age and situation, including discharge plans.
- Revised treatment plans based on patient progress towards stated goals.
- Document patient information, treatment plans, and progress in accordance with facility and practice guidelines.
- Orient, train, and mentor therapy and health care students to achieve stated performance expectations.
- Identify and act upon performance improvement opportunities to implement best practices.
- Master of Science in Physical Therapy preferred; Bachelor's degree required.
- Valid licensure as a Physical Therapist in Massachusetts required.
- Current CPR certification required.
- Experience in ambulatory and acute care settings required.
