Specialty Pharmacy Patient Care Coordinator Job Description

This role is critical in providing comprehensive support to patients, pharmacists, and clinic staff in the delivery of specialty medication services. The successful candidate will possess strong administrative and organizational skills, as well as a focus on delivering high-quality patient care and service.

Main Responsibilities:
  • Integrated support to patients, pharmacists, and clinic staff throughout the full continuum of specialty medication services.
  • Maintenance of high levels of accuracy and attention to detail in all aspects of work.
  • Strong administrative and organizational skills, ensuring efficient and effective workflow.
  • A patient-centered approach to care, prioritizing exceptional customer service.
  • Collaboration with healthcare professionals to optimize patient outcomes and ensure seamless transitions of care.
Key Requirements:
  • High School diploma or equivalent, plus one year of post-secondary education or related work experience.
  • Pharmacy Technician National Certification through PTCB or NHA.
  • Vermont Board of Pharmacy Technician License.
  • Maintenance of Pharmacy Technician Registration/License with the State of Vermont Board of Pharmacy and National Certification in good standing.
  • One year of experience as a Pharmacy Technician or equivalent required; three to five years preferred, with specialty pharmacy or pre-certification experience strongly encouraged.

Salary Range: $60,000 - $80,000 per year
