Position: Senior Adult Counselor
Compensation: $ unlicensed, pay rate dependent on program)
Role Overview: As a Senior Adult Counselor at Aspire Health Partners, you will play a crucial role in delivering a range of vital services to individuals enrolled in our programs.
Key Responsibilities:
- Facilitating therapeutic sessions in both individual and group formats for participants.
- Developing, revising, and tracking participant treatment plans to ensure effective care.
- Conducting program-specific evaluations to assess participant needs.
- Recording treatment details accurately in the Electronic Health Record system.
- Maintaining ongoing communication with the program team regarding participant progress, challenges, or requirements.
- Supporting the program team with operational tasks as needed.
- Coordinating appropriate discharge planning and referrals for participants.
- Ensuring compliance through completion of required training and participation in staff meetings.
- Performing additional duties as assigned.
Required Qualifications:
- Master's Degree in a relevant social services field.
- Preferred: At least 1 year of experience working with the target population.
- Must pass a Level II Background clearance.
