POSITION OVERVIEW
The Service Coordination Specialist plays a vital role in advancing the mission and core values of BDI, Inc.

by facilitating service coordination in alignment with the Department of Behavioral and Developmental Disabilities (DBHDD) contract. This role focuses on identifying, coordinating, and supervising the provision of services that promote the health, safety, and overall well-being of individuals with developmental disabilities, guiding them towards optimal independence.

KEY RESPONSIBILITIES

  • Conduct comprehensive Needs Assessments for individuals to connect them with essential services, community resources, and support networks (both in-person and virtual).
  • Lead the development of Individualized Family Support Service (IFSS) plans in collaboration with participants and their families to evaluate and strategize available services.
  • Deliver support coordination services to individuals requiring critical health and safety services and supports.
  • Evaluate, coordinate, broker, plan, and/or link individuals and families to direct services, acting as a liaison between them and service providers.
  • Advocate for participants to secure necessary services or benefits.
  • Provide transitional assistance for individuals moving from the IFSS program to other services once they are stable.
  • Authorize and oversee financial management for goods and services.
  • Maintain detailed progress notes and ensure contact with enrolled individuals at least twice a month during IFSS services, documenting updates on family situations, improvements, barriers, etc.
  • Facilitate virtual meetings, support sessions, training, webinars, and resource presentations (both in-person and virtual).
  • Participate in regional field office provider meetings.
  • Perform other duties as assigned.

EDUCATION AND EXPERIENCE REQUIREMENTS

  • Minimum of one year of experience in direct client services.
  • At least one year of experience working with individuals with developmental disabilities.
  • Bachelor's Degree in Social Work, Psychology, Rehabilitation Counseling, or a related field.

PREFERRED QUALIFICATIONS

  • Bachelor's Degree in Social Work, Psychology, Rehabilitation Counseling, or a related field.
  • Certification as a Rehabilitation Counselor or Vocational Evaluator is preferred.
  • Licensure as an Independent Clinical Social Worker (LCSW) or Licensed Professional Counselor is preferred.

ADDITIONAL QUALIFICATIONS
Successful completion of background checks is required.
