Job Summary

The Case Manager RN ensures high-quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes. This role provides, coordinates, and directs care specific to the needs of each Rehab patient.

Responsibilities
  • Provide program orientation to patients/families/caregivers, including the case manager's role, Rehab philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences.
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient/family/caregiver.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
Requirements
  • Associate Degree in Nursing or Nursing Diploma required; Bachelor's Degree in Nursing preferred.
  • 2+ years experience in case management OR 3+ years experience in clinical nursing required.
  • Current license as a Registered Nurse in the state(s) of practice according to law and regulation.
  • InterQual experience and Certification in Case Management preferred.
  • Basic Cardiac Life Support BCLS required.
