- MD or DO degree or foreign equivalent.
- Eligibility to obtain state medical license.
- Completion of a Family Medicine, Internal Medicine, or Med-Peds residency program, along with a Sports Medicine fellowship.
- Board certification/eligibility in Family Medicine, Internal Medicine, or Med-Peds.
