About the Role

LHC Group is seeking a skilled Physical Therapist to join our Home Health team in Berwick, Louisiana.

As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.

You will work closely with the patient, family, and healthcare team to develop and implement a personalized treatment plan, perform interventions to improve and enhance the patient's well-being, and evaluate progress.

Responsibilities
  • Evaluate patient's functional status and physical therapy needs.
  • Assist physician in developing therapy plan of care.
  • Observe and report patient's response to treatment and any changes in condition to supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals.

Requirements
  • Current Physical Therapy licensure in state of Louisiana.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to dependable transportation.
