Job Title: Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Salinas Valley Post Acute in Salinas, CA. As a Physical Therapist, you will play a vital role in helping our patients achieve their rehabilitation goals.

Job Summary:

The Physical Therapist will be responsible for developing and implementing effective treatment plans for patients, working closely with the healthcare team to ensure seamless care. This includes:

  • Developing and implementing treatment plans in collaboration with referring physicians
  • Treating patients according to physician-approved treatment plans
  • Assisting the nursing department with training of Restorative Aides
  • Supervising Physical Therapy Assistants in direct patient care and patient-related activities
  • Communicating with the supervisor and other healthcare team members regarding patient progress, problems, and plans
  • Participating in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed
  • Participating in in-services training programs for other staff in the facility
  • Recording evaluations, daily treatment notes, weekly progress notes, recertification documentation, and treatment changes according to policy and procedures
  • Instructing patient families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services
  • Participating in discharge planning
  • Recommending and facilitating the ordering of necessary durable medical equipment for patients to facilitate independence in mobility
  • Reporting any problems with department equipment to ensure it is maintained in good working order

To be successful as a Physical Therapist at Salinas Valley Post Acute, you must have:

  • A valid Physical Therapist license
