Children's Health Home Care Manager

We are seeking a highly skilled Children's Health Home Care Manager to join our team at Mary Cariola Center. As a key member of our staff, you will be responsible for providing high-quality care management services to children and families in their homes, schools, and communities.
Key Responsibilities- Provide outreach and enrollment services to children and families
- Develop and implement Plans of Care and Individualized Family Service Plans (IFSPs)
- Coordinate and secure services relevant to the Plan of Care and IFSP
- Complete documentation with a high level of quality and detail to meet and exceed Health Home, Early Intervention, and Department of Health requirements
- Participate actively in trainings, meetings, and functions at the agency and in the community
- Provide quality direct care management services to children and families in home, school, and community settings
- Use a strength-based approach to arrange for, educate about, and manage the coordination of medical care and other services as needed
- Provide on-call availability to the Health Home Program as assigned
- Interface and network with partnering Health Homes, providers, and agencies
- Bachelor's degree in Human Services, Nursing, or related field required; Master's degree in Human Services or Nursing preferred
- Minimum of one to three years related care management experience with persons with disabilities and/or chronic illnesses
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information
- A competitive salary range of $20.00 - $24.00 per hour
