Key Responsibilities- Provide direct patient care in accordance with established policies, procedures, and protocols of the healthcare organization.
- Implement and monitor patient care plans, ensuring seamless coordination with other healthcare disciplines.
- Monitor, record, and communicate patient condition as necessary, maintaining accurate and detailed records.
- Serve as a primary coordinator of all disciplines, ensuring well-coordinated patient care and exceptional patient satisfaction.
- Note and carry out physician and nursing orders, prioritizing patient needs and safety.
- Assess and coordinate patient discharge planning needs with members of the healthcare team, ensuring a smooth transition to post-acute care.
- BLS Provider certification obtained within 1 month of hire date required.
- Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date required.
- Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
